Am I Suppose To Fill Out The Health Form And Send It Out In The Mail Or Can I Just Do It Online.

And Whats The Differene Between Doing It Online Then Sending It In The Mail.

There are 2 requirements for Immunizations. The Meningitis form can be done online. The link is https://bscintra.buffalostate.edu/Weigel/MeningitisForm.htm
You don't have to get the meningitis immunization, but you do have to tell us on the form if you've gotten it or not. If you did get it, we do need a date (month & year is enough).
The other requirement is for measles, mumps & rubella. For this, we need an official record, such as from your school or from your doctor. If you use the New Student Health Form, your doctor has to complete the form and sign it.
