i have health insurance through the place i work. where do i get the address for my health insurance and the phone number? do i just get it from the bills i receive or what? thats where i go to fill out the form right? https://www.consolidatedhealthplan.com/stud...ege/waiver.html and how long do i have to fill it out? i'm only going to be part time so do i even need to fill it out?

Part Time students do not need to complete the online waiver as they are not required to have health insurance.

If you want to get the your health insurance address and phone number, then you can go to consolidatedhealthplan online site here they will provide an all instruction about your health insurance with health insurance company's address and phone number.
