According to New York State Public Health Law and Buffalo State College policy, students are required to provide proof of immunization. The requirement is that students have received 2 measles (the first after their 1st birthday and the 2nd, atleast 30 days after the first shot), 1 mumps and 1 rubella immunization. Students can also have received 2 combination shots (measles, mumps & rubella) MMR, the 1st after their 1st birthday and the 2nd atleast 30 days after the first shot.
New students receive the "New Student Health Form. This must be signed by a heath care provider or have an official record attached to it. Official records can be obtained from a doctor or from a prior school (either college or high school). Meningitis Information is also required, but can be self report either on the New Student Health Form or on line at buffalostate.edu/weigel. |
